Judith,

Were you referring to parent/carer other family member disabilities or 
the pupil?

In both cases  I tend to talk direct to parents I see/know about but 
sometimes pupils have a disabled parent who cannot or does not attend 
school and it tmay be difficult to find out this information in order 
to help..

I was wondering how people had decided to gather disability information 
from parents?

Has anyone written a sensitive letter to gather this information and 
willing to share.
