The borough I work in now uses SEN Management forms, as did my previous borough. You record all the information from SIMS re previous schools, professionals involved if SA+, dates of IEP reviews and the outcome, and end of KS results for FS, KS1 and KS2. I amended ours to include all tracking information, after I saw another school's one at an SEN file moderation day. It's the first page in the pupil's SEN file.
